Adult Psychiatric Rehabilitation Program (PRP)

Adult PRP services are available to those individuals who are 18 years of age and older, who are receiving mental health services from a physician or licensed clinician, who have or are eligible for medical assistance, and who meet criteria for a priority population mental health diagnosis.  Services are offered in multiple settings/combination of settings in order to best meet the needs of the client (i.e. onsite day program, offsite outings, in-home, etc.).  Following a period of assessment which evaluates client strengths and preferences, individualized rehabilitation goals are implemented.

Target areas may include but are not limited to the following:

  • Development of natural supports through increased opportunities for socialization
  • Connection to community resources
  • Navigation of the entitlement system (i.e. SSA, DSS, local housing agencies)/ Case management
  • Advocacy
  • Life skills training (i.e. grooming, hygiene, nutrition, dietary planning, budgeting/money management, self-administration of medication)
  • Development of crisis management and wellness self-management/recovery related skills.
  • Assistance with scheduling and maintaining appointments.
  • Coordination of care with other providers/treatment team members (i.e. therapist, psychiatrists, PCP, substance abuse providers, specialists).
  • Mobility skills training (i.e. assistance with applying for various forms of reduced cost transportation, assistance with understanding the bus system, etc.)
  • Other areas of need as identified
